The aim of all support and care provisions is to ensure that the right level of service is provided at all times. Through on-going assessment, monitoring, review and consultation, the service provision will adapt with the changing needs of the Service User.

Through constant research, professionals meetings, membership of specialist bodies, on-going training and Service User involvement, we aim to ensure our service and accommodation provisions are compliant, meet our Service Users' needs, provide realistic expectations and achieve positive outcomes.

   
Supported Living
   
 

This project provides a comprehensive service for adults with mental health, autism and learning disabilities. Following a full assessment and preparation of a care plan, care packages are tailored around the clients needs. The aim is to provide an environment as near as possible to independent living, whilst benefiting from the availability of unobtrusive support and care as required, to develop personal, social and domestic skills.

Those placed within this scheme will usually live within a small group of well appointed units and will be supported 24 hours a day by staff available within the same building and some properties have disabled access and wheelchair accessible rooms and communal areas.

This well established Supported Living arrangement has proven to be the most successful, as it also promotes, encourages and achieves a feeling of privacy, dignity and independence to the Service User. Under the Registered Manager, each person’s accommodation is fully furnished, benefits from all modern facilities and is well located to enable easy access to amenities.

Supported Housing
   
 

This service provides accommodation and support services for young people aged 16-24, including those leaving care, unaccompanied asylum seeking children and teenage parents. Following a comprehensive needs and risk assessment and the preparation of a support plan, the objective of the service is the progression of the young person to “move on” and live independently.

Our in-house link worker/out reach team focus on matters such as life skills, identifying and sourcing education/training, accessing health and benefits and are available 24 hours a day, including weekends.

Monitored Accommodation
   
 

Monitored accommodation and support services are provided for individuals aged 16 years old onwards, with a variety of needs. The support and accommodation services are usually provided on basic three levels, but are flexible and adaptable to meet the service users needs:

Low Support Service: This accommodation is regularly monitored by a floating team. The monitoring team will undertake planned, routine and reactive visits each week, including out of hours and weekends. Each Service User receives up to four hours of 1:1 key work support each week.

Medium Support Service: This level of service provides a greater and more structured staffing presence within the accommodation, including in the evening and weekends. Each Service User usually receives a minimum of seven hours of 1:1 key work support each week.

Staffed Accommodation
   
 

Unlimited additional support hours are available as required, including floating support for off site activities. Working with a number of Local Authorities, accommodation and support services can be tailored to fit the individual person's needs, based on the referral information, assessment and person centred planning process.

High Support Service: This service is designed for Service Users assessed as requiring the highest level of need and is usually provided within 24 hour staffed accommodation. Each Service User usually receives a minimum of fourteen hours of 1:1 key work support each week.

With over 40 members of experienced staff and registered managers, we are able to support service users with a variety of needs, including those with mental health issues, learning disabilities and/or challenging behaviour.

Domiciliary Care
   
 

This specialist team provides support and personal care for adults with mental health issues, autism, learning difficulties and some challenging behaviour, following a full assessment and preparation of a care plan. Suitable accommodation can also be sourced and support packages are tailored around the clients needs.

A full range of activities are offered within and away from their home and all our Service Users are given the opportunity and encouraged to engage in a wide range of social activities/hobbies etc. Care packages are tailored to suit individual needs, as agreed between the placing authority, other stakeholders and ourselves and are regularly monitored, in conjunction with the Individual Programme of Assistance/Care Plan.

Home Based Support and Care Services
   
 

This service provides the help required to continue living a free and independent life, in your own home. Professional home carers are available and will visit your home as little or as often as needed. After an assessment of needs, a schedule and care plan will be provided to suit you, that can include household chores, assistance with washing, dressing and food shopping to more demanding, personal and intensive care needs.

Following an enquiry, a free Home Care Evaluation will be undertaken to ensure that the right service is provided at the right time and that all the terms and conditions are full understood and agreed. A number of flexible options are available, so that the care can range from one off support to a full time total provision and because of the quality of our skilled, attentive and specialist carers we can provide home care for a wide range of people, including those with learning disabilities, mental illness, physical disabilities and sensory impairment.

Personalisation and Care Management
   
 

For some care services, the Government is leaning towards self directed care, where the Service User receives and controls their own care budget, so that they can put together their own care package and make their own budget choices.

For some people, this is a major change to the way their care services will be managed and financed and our specialist team is able to provide assistance, advice and guidance to help identify and source the most effective, relevant and cost efficient support and care required. Our team can manage the whole process and also offer more specific support for a particular service, as required.

Temporary & Long Term Housing Solutions
   
 

The range of accommodation provided includes both shared and self contained properties and is available throughout the North London area, predominantly in Enfield, Haringey, Barnet and Waltham Forest.

To support our tenants and service users, we provide a 24 hour property management and maintenance service and a wide range of specialist housing related and tenancy sustainment support services.

Move On Services
   
 

Independent living is an aim of the majority of Service Users, to varying degrees. Our support and care teams will assess all Service Users and make this a focal point of the support/care plan. Our specialist property services team are able to source a wide range of accommodation, to suit the needs of most Service Users, with support levels adjusted and gradually withdrawn, as appropriate.

Bespoke Services
   
 

The support and care teams include a diverse range of staff, able to address the needs of most people. Projects can be set up for individuals or groups of people and we have a varied portfolio of properties available, many of which can be altered to suit specific Service Users needs. This can range from changing colours and furnishings to being adapted to facilitate a disabled occupant.

Oak Tree Care Services is not only an accommodation based support service and floating support is available and provided to eligible people, as required and agreed. The support team is independently managed and works in partnership with the housing team/provider to meet the service user’s needs and achieve positive outcomes, with the overall aim to progress the service user to “move on” and live independently.
